Microsoft currently ships two different Outlook apps side by side, and they set up signatures differently - pick the section that matches what you're running.

Note: Like most mobile mail apps, the Outlook app's signature field is plain text only. For a fully-styled signature with logos and fonts, set it up in desktop Outlook instead.
They're separate apps with separate signature settings - a signature saved in Classic Outlook doesn't carry over to New Outlook (or Outlook.com) and vice versa. Set it up in whichever version you actually send mail from.
Yes. In Classic Outlook, assign a default signature per account under "Choose default signature." In New Outlook, pick the account first at the top of Accounts > Signatures before creating the signature.
Outlook sometimes defaults to "Keep Text Only" on paste. Right-click the pasted content and choose "Keep Source Formatting" (Windows), or use the formatting pop-up menu that appears after pasting (Mac).
Only if you set it. The signature settings have separate dropdowns for new messages and for replies/forwards - point both at your signature if you want it everywhere.
